口腔种植Onlay植骨技术的应用

【摘要】  目的  研究探讨通过Onlay(上置法)自体骨移植术后牙槽嵴水平向和垂直向骨量变化;评价后期种植体植入的临床效果,以及该项技术在口腔种植中的应用。方法  2015年9月至2017年10月,我郑州大学附属口腔医院的种植科5位牙齿缺损患者为研究对象,根据术前口腔专科检查和CBCT检查的术前评估,制定合理的Onlay自体骨移植和种植手术方案,恢复缺牙区的骨量和缺失牙,手术后通过CBCT检查来检测牙槽骨骨密度的变化。结果  在5例6个月后通过Onlay自体骨移植术后CBCT检查在牙槽骨骨密度的变化,发现缺牙区牙槽嵴水平高度平均上涨了4.3毫米的骨量,垂直向骨高度平均上涨了3.19毫米垂直骨量;种植后4 - 6个月,存活率100%。结论  Onlay自体骨植骨术是能够解决中度甚至重度骨萎缩或者是骨缺损的有效临床途径,能够做到牙槽骨水平向及垂直向骨量都得到良好的改善。

Application of oral implanting Onlay bone grafting technology

Abstract  Objective  To study changes of horizontal and vertical alveolar bone mass after autologous bone grafting with Onlay, and to evaluate the clinical effect of implant implantation in the late stage and the application of this technique in dental implants.Methods  Five tooth defect patients dental treated with implants in Dental Department of Stomatology Hospital Affiliated to Zhengzhou University from September 2015 to October 2017 were studied. A reasonable Onlay autologous bone graft and implant surgery program were planned according to the preoperative assessment of the mouth and CBCT before surgery to restore the bone loss in the edentulous area and missing teeth, density alteration of alveolar bone after surgery was detectd by CBCT.Results  After 6 months of oral implanting Onlay bone graft for the 5 cases, density alteration of alveolar bone after surgery detected by CBCT found that the alveolar crest level in the edentulous area increased by an average of 4.3 mm and the mean vertical bone height increased 3.19 mm vertical bone mass; 4 to 6 months after planting, the survival rate was 100%.Conclusion  Onlay autograft is an effective clinical approach to solve moderate or even severe bone atrophy or bone defect. It can improve the horizontal and vertical alveolar bone mass well.
