| 心衰易损期患者院外管理的价值 【摘要】目前，心力衰竭（心衰）作为多种心血管疾病的终末阶段，因其发病率逐年上升、治疗费用居高不下，且5年存活率与恶性肿瘤相仿,已成为本世纪最重要的心血管病症。研究表明：心衰易损期患者出院后1-3个月死亡率及再入院率分别高达15%和30%。目前对于心衰院内治疗的措施相对完善，而对于出院后短期(即易损期)的管理手段却很是有限，导致患者出院后的转归并不理想。本研究利用心衰院外监护系统对心衰易损期患者进行远程监测，并通过定期门诊随访，观察心衰院外远程监测系统在心衰易损期患者中的预测价值，同时使医生对患者的疾病状况和发病趋势得到了解与预测，达到早期发现、早期评估、及时干预的目的。 【关键词】心衰易损期 院外远程监测 预测 干预 【Abstract】 At present, heart failure (CHF) as the end stage of many cardiovascular diseases, its incidence increased year by year, the high cost of treatment, and the 5 year survival rate is similar to malignant tumor, has become the most important cardiovascular diseases this century. The study shows that heart failure patients vulnerable period 1-3 month post discharge mortality and readmission rate were as high as 15% and 30%. for the treatment of heart failure in the hospital measures relatively perfect, and for the short term after discharge (i.e. vulnerable period management) is very limited, resulting in prognosis of patients after discharge is not ideal.
